Services may be covered in full or in part by your health insurance or employee benefit plan so please be sure to contact them for exactly what you are covered for including deductibles and copays before you schedule an appointment with us. If your insurance is not on the list above, check to see if your health insurance carrier allows you to choose an out-of-network provider and if they do, we will assist you with obtaining your reimbursement.
If we do not participate with your insurance and finances are a barrier to treatment, we may be able to offer services on a sliding fee basis.
Cash, credit card, or personal check are accepted for payment.
